Texas HHSC-Annual Prevention Providers’ Meeting, June 21-24

Session Description: Communities of Care: Insight on Community Connectedness and Collaboration amidst COVID-19Our session will provide an overview of current community level prevention work that is being led by 10 diverse collaboratives across Greater Houston that are made up of civic and faith groups, middle and high school partners, health clinics and nonprofits, as well as parents and youth of color. The presentation will detail the ongoing efforts of these groups in the ways they’ve addressed community connectedness in the context of the COVID-19 pandemic. Our session will also describe how similar prevention work through a community of practice can be replicated in other contexts (e.g. cities, counties, schools, healthcare providers) and what all is required to coordinate and fund this endeavor.
